apps/api/tactile-api/src/middleware/rate-limit.ts implements a per-account
sliding-window limiter over a Durable Object, applied globally at
index.ts:425 (`const limited = await applyRateLimit(request, env);`) AHEAD of
the route dispatcher, for every method other than GET/HEAD/OPTIONS.

Five individually reasoned public exemptions (Stripe webhook, health, public
form submit, abuse intake, destination engagement events + the open beacon),
each applying its own per-IP window instead.

NOT a production gap: the limiter fails OPEN when env.RATE_LIMIT_DO is unbound
(rate-limit.ts:22-23), deliberately, for dev/CI — and the binding IS present in
wrangler.toml for both prod (:127) and [env.staging] (:348). Verified in the
tree, not assumed.
